Gun violence restraining order forms

Gun Violence Restraining Order After Hearing or Consent to Gun Violence Restraining Order (CLETS-OGV) GV-130 If the judge grants the restraining order, they will sign this form.  
Proof of Service by Mail GV-250 Use to prove that you had someone 18 or older (not you) mail the restrained person a copy of the Gun Violence Restraining Order After Hearing (Form GV-130). Make sure you can in fact serve Form GV-130 by mail in your case. Serve your gun violence restraining order
