📘 Informational form: How to Adopt a Child in California

ADOPT-050-INFO This form tells you about adoptions. You don't fill it out.
🟥 Required form: Adoption Request ADOPT-200 This form tells the judge about you and your child.
🟥 Required form: Adoption Agreement ADOPT-210 This form tells the judge that you and your child (if they are 12 or older) agree to the adoption. Fill it out, but wait to sign it until the judge tells you to.
🟥 Required form: Adoption Order ADOPT-215 The judge signs this form to show that your adoption is approved.
🟥 Required form: Indian Child Inquiry Attachment ICWA-010(A) This form tells the judge that you asked if the child may be an Indian child.
🟥 Required form: Parental Notification of Indian Status ICWA-020 You must fill out one form for each birth parent. This form shows that each parent was asked if the child may be an Indian child.

🟧 If needed form: Adoption of Indian Child

ADOPT-220 This form tells the judge about you and your Indian child.
🟧 If needed form: Parent of Indian Child Agrees to End Parental Rights ADOPT-225 This form tells the judge that the Indian child's birth parents agree to end their parental rights.
🟥 Required form: Adoption Expenses ADOPT-230 This form lists the costs you paid for your child's adoption.
🟧 If needed form: Contact After Adoption Agreement ADOPT-310 This form tells the judge how your child's birth family can stay in touch with your child.
🟧 If needed form: Request to: Enforce, Change, End Contact After Adoption Agreement ADOPT-315

This form tells the court that contact after the adoption isn’t happening, or that it needs to change or stop. You can only use this form after everyone in the agreement has tried to fix the problem with a mediator or another dispute resolution program.

🟧 If needed form: Answer to Request to: Enforce, Change, End Contact After Adoption Agreement ADOPT-320 This form tells the court and the other side that you got a copy of the Contact After Adoption Agreement (form ADOPT-310) and the Request to: Enforce, Change, End Contact After Adoption Agreement (form ADOPT-315). It also says whether you agree or disagree with the request.

ADOPT-325 This form shows the court’s decision to enforce, change, or end the contact after adoption agreement.

📘 Informational form: Information Sheet on Indian Child Inquiry Attachment and Notice of Child Custody Proceeding for Indian Child ICWA-005-INFO This sheet gives information to help you fill out forms to find out if a child may be an Indian child. If so, it explains how to give formal notice about the custody proceedings.
🟥 Required form: Indian Child Inquiry Attachment ICWA-010(A) This form tells the judge that you asked if the child may be an Indian child.
🟥 Required form: Parental Notification of Indian Status ICWA-020 You must fill out one form for each birth parent. This form shows that each parent was asked if the child may be an Indian child.
🟧 If needed form: Notice of Child Custody Proceeding for Indian Child ICWA-030 This form tells an Indian tribe that a child in the case may be a member. It includes family background information so the tribe can decide if the child is a member or eligible to become one.
🟦 Optional form: Attachment to Notice of Child Custody Proceeding for Indian Child ICWA-030(A) Use this attachment with form ICWA-030 to list any extra tribes or bands.
🟦 Optional form: Notice of Designation of Tribal Representative and Notice of Intervention in a Court Proceeding Involving an Indian Child ICWA-040

An Indian tribe uses this form to tell the court who can represent the tribe in a case involving a child who is a member of that tribe.

🟦 Optional form: Notice of Petition and Petition to Transfer Case Involving an Indian Child to Tribal Jurisdiction ICWA-050

An Indian tribe, a parent of an Indian child, or an Indian custodian uses this form to ask the court to move a custody case to the tribe’s court.

🟧 If needed form: Order on Petition to Transfer Case Involving an Indian Child to Tribal Jurisdiction ICWA-060

The court uses this form to record its decision on a request to move a custody case for an Indian child to tribal court.
