Attachment to Notice of Hearing Proof of Personal Service (DE-120(PA))

Gives more space to list people who had a Notice of Hearing delivered in person to (personally served on) them. This can be attached to either Proof of Personal Service of Notice of Hearing—Decedent's Estate or Trust (DE-120(P)) or Proof of Personal Service of Notice of Hearing—Guardianship or Conservatorship (GC-020 (P)).
