Proof of Service of Order to Show Cause (NC-121)

Tells the court  that you had the Order to Show Cause and other name change forms delivered to (served on) the other parent or relatives in a case. Lists the papers that were served and tells who they were served on, where, when, and how they were served, and who served them.
