Continuation of Notice to Appear (TR-106)

A law enforcement officer uses this when a Notice to Appear form does not have enough space to list all the charges. This can be attached to a Nontraffic Notice to Appear (form TR-120) or a Traffic/Nontraffic Notice to Appear (form TR-130).
