Child support forms

All California courts use the same basic set of forms. You can find the most commonly-used forms on this page. If you are not sure which forms to use, find out from your court or talk to your self-help center

Ask for a child support orders

FL-300

Use this form to ask the court to make decisions (orders) about specific issues in your family law case.  You must already have an open family law case or be opening one at the same time. Ask for or change child support

Income and Expense Declaration

Gives your financial information to the court and to your child's other parent.   
Proof of Personal Service FL-330 Tells the court that you had the other party served in person with legal papers. Serve your request in person
Proof of Service by Mail FL-335 Tells the court that you had the other party served legal papers by mail. Serve your request by mail
Responsive Declaration to Request for Order FL-320 If you're asking for child support, you give the other side a blank copy of this form. The other parent would fill it out to tell the court and you if they agree or disagree with the orders you asked for in Request for Order (form FL-300).

Declaration Under Uniform Child Custody Jurisdiction and Enforcement Act 

FL-105/GC-120

 

Gives the court important information about the children you have with your child's other parent.  
Attachment to Declaration Under Uniform Child Custody Jurisdiction and Enforcement Act FL-105(A)/ GC-120(A) Attachment to form FL-105 if you have more than 2 children with your children's other parent.

Gives your financial information to the court and to your child's other parent.  
Proof of Service of Summons FL-115 Tells the court that you had the other party served with the legal papers to start your case.   
Response to Petition for Custody and Support of Minor Children  FL-270 If you're opening a case to ask for child support, you give the other side a blank copy of this form. The other parent would fill it out to tell the court and you if they agree or disagree with the orders you asked for.

Set up or change a support order

Explains how to ask the other parent to help pay for your child's health care cost that is not covered by insurance.  

Child Support Case Registry Form

FL-191

Gives information to the court about the child support or family support order in your case.  
Income Withholding for Support FL-195 Tells an employer that the court made an order for you or the other person to pay child support and to garnish the wages to pay the support amount.
