Elder and dependent adult abuse forms

All California courts use the same basic set of forms for elder and dependent adult abuse restraining orders. You can find the most commonly-used forms on this page. If you are not sure which forms to use, find out from your court or Self-Help Center.

Some courts have special local forms too. To see if you will need any special local forms, contact your court clerk or check your court’s website.

Proof of Personal Service EA-200 Use to prove that you gave the person to be restrained a copy of the petition, notice of court hearing and temporary restraining order in person. How to serve the request
Proof of Service of Order After Hearing by Mail EA-260 Usually, the protected person must serve papers in person, but in some situations, the restraining order after hearing, on Form EA-130, you are legally allowed to serve by mail. In that case, you can use this form. Serve elder or dependent adult abuse restraining order
Elder or Dependent Adult Abuse Restraining Order After Hearing (CLETS — EAR or EAF) EA-130 If the judge grants any restraining orders, they will sign this form.  
Response to Request for Elder or Dependent Adult Abuse Restraining Orders EA-120 Leave blank and serve on restrained person with the TRO (Form EA-120).  
Proof of Firearms Turned In, Sold, or Stored EA-800 Leave blank and serve on restrained person with the TRO (Form EA-120).
