Workplace violence restraining order forms

All California courts use the same basic set of forms for workplace violence restraining orders. You can find the most commonly-used forms on this page.

Ask for a workplace violence restraining order

Civil Case Cover Sheet	CM-010	Required when you file any new civil case.
Declaration OR Attached Declaration	MC-030 OR MC-031	Use to give more information or for witness statements.
Proof of Personal Service	WV-200	Use to prove that you gave the person to be restrained a copy of the petition, notice of court hearing, and temporary restraining order (if issued).	Sheriff serves your request or Serve your long-term workplace violence restraining order
Workplace Violence Restraining Order After Hearing (CLETS-WHO)	WV-130	If the judge grants any restraining orders, they will sign this form.
Response to Petition for Workplace Violence Restraining Orders	WV-120	Leave blank and serve on the person to be restrained along with the Petition.
Proof of Firearms Turned In, Sold, or Stored	WV-800	Leave blank and serve on person to be restrained along with the Petition.
